WHAT I DID

The review showed that the incident was not isolated. It exposed wider gaps in procedures, contractor management, training, reporting and corrective action tracking across all sites in the network.

This led to a prioritised programme of system improvements, clearer responsibilities and stronger operational controls. It also brought the psychological impact on witnesses into the response, supporting practical psychosocial guidance and worker support. The focus shifted from simply closing out a notifiable incident to addressing the organisational conditions that could allow another serious event to occur.

THE ISSUE

THE OUTCOME

I reviewed policies, procedures, safe operating procedures, training and competency requirements, contractor compliance, audits, incident follow-up, safety reporting and governance across a large, multi-site branch network.

I then migrated the complete health and safety management system into a digital health and safety system (Safe365), centralising documents, registers, reporting, audits and corrective actions. I also developed contractor management and induction processes and introduced tablet-based visitor sign-in to strengthen site access, emergency visibility and recordkeeping.

I worked with operational teams to strengthen systems and develop practical resources for a diverse workforce, including workers with English as an additional language.

I also raised an issue receiving far less attention: the psychological impact on workers who witnessed the incident. I helped broaden the response beyond physical safety, regulator requirements and closing corrective actions to include trauma exposure, psychosocial risk, worker wellbeing and organisational recovery.

I joined a large industrial recycling organisation approximately three weeks after a major notifiable incident witnessed by several workers.

The organisation needed to respond to immediate regulatory and physical safety requirements while also examining the systems, procedures and organisational conditions surrounding the incident.

Key Learning

A serious incident response must address organisational failures and the psychological impact on witnesses, not only physical controls and regulator actions. Corrective actions need to be embedded through systems, training and worker support.
